Summary

  • Basic Description
    • Responsible for managing the medication use process - prescribing, dispensing, administration, and monitoring - throughout the medical center, and for applying advanced knowledge about drugs and drug therapy to the care and treatment of patients. Provide advanced clinical services independently and promotes pharmacy department clinical development.
  • Specialized Knowledge, Experience and Skills

    • Degree / Education
      • Bachelor Degree PHARMACY
      • OR
      • Doctor of Pharmacy
    • Level of Experience
      • 2 year residency required for ID, Critical Care, Oncology, and Peds positions OR 1 year residency required for Internal Medicine and Ambulatory Care position
    • Lic./Cert./Reg.
      • All licensure must be current/valid and from the state of West Virginia.
      • Pharmacist, Licensed
      • Basic Life Support - Must be certified within 30 days of hire.
      • Advanced Cardiovascular Life Support - Must be certified within 1 year of hire.
      • Pediatric Advanced Life Support - Must be certified within 1 year of hire.
    • Typing/Word Processing skills required for this position
    • Computer Skills required for this position

        • Eligible to sit for Pharmacotherapy specialty certification for practice areas without an established specialty credential
    • Patient Group Knowledge (Only applies to positions with direct patient contact)
      • The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department’s identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.
